Forum at the Crossing features 118 Independent Living apartments, 30 Alzheimers/Memory Care suites and 74 Skilled Nursing and Rehabilitation beds.
The Forum is for seniors wanting to live life to the fullest without the stresses and bother of maintaining a home. Residents choose from a variety of floor plans and amenities such as transportation and concierge services, fine dining and social and recreational activities including musical programs and other entertainment.
Housekeeping and flat linen services and wellness programming are also part of the offer. A full-service beauty and barber shop is on-site as is a convenience store and gift shop. Maintenance and security are provided and utilities excluding phone. There is a 24 hour emergency call system and Wifi access is also available.
Those residents requiring help with every day activities such as setting up medications, bathing and dressing can access enhanced services. Tailored to the unique needs of each resident, assisted living services are part of care plans that are assessed regularly.
Using the Five Star corporations Bridges to Recovery program, residents with Alzheimers or any other memory or cognitive impairment are encouraged to participate in daily activities by specially trained staff who are on hand around the clock to help those in their care to better engage their surroundings. Inspired by the Montessori approach to educating children, the program begins with learning each residents life story and personal care requirements.
Activities have clear objectives, are meant to be meaningful to the resident and are provided in an atmosphere of acceptance. Families are encouraged to learn and add to the program through meaningful visits. Memory Care suites feature familiar furniture, contrasting colors and other visual cues to stimulate memory and help orient residents to their surroundings. There is a specialized food and beverage program and electronic door security for maximum safety inside and out.
Skilled nurses are on staff and are available to long term residents and those who do not live on-site but are also recovering from surgery, illness or who need specialized rehabilitation care. Nurses provide care plans working with each resident, their family and personal physician. Short term stays for rehab and caregiver respite are offered in the Forums apartments and health care center based on availability.
Outpatient services also include physical, occupational and speech and language therapy.
